You’re ready to live larger. You’re ready for Willamette View. What now? Any move is easier when you have a checklist. Ours will help get you started.

Revise advance directives
Each state has its own laws regarding advance directives, so take special care to follow the laws of the state where you will be living. It’s possible that a living will or medical power of attorney signed in one state may not be recognized in another. Following your move to Oregon, it is important to revise your advance directives. An attorney is not required for this process.

Make sure you have a living will
Requirements for a living will vary by state so you may want to have a lawyer prepare one for you. Many lawyers who practice in the area of estate planning include a living will and a medical power of attorney in their package of estate planning documents. If you need to write or update a will or trust, you can take care of your living will at the same time.

Complete medical power of attorney form
It is important that you complete the medical power of attorney form that sets forth the information required in the state of Oregon. This information varies slightly from state to state. The legal status of the medical power of attorney signed in one state may not be fully recognized in another.

Look for a new medical provider
Moving to Oregon will require you to select a new Primary Care Physician (PCP). We suggest you ask for referrals from other residents at Willamette View, or from a friend or family member who lives in Portland. Friends and family often are quick to volunteer their opinions about providers. You also could ask your current physician for help in locating a new provider in the Portland market.

One factor in your choice is likely to be the provider’s proximity to Willamette View. If you or your family would prefer a provider nearby, Adventist Health Medical Group operates an established primary care clinic within The Plaza building on the Willamette View campus. An MD and Nurse Practitioner are available Monday through Friday during business hours.

In addition, there are several off-site medical facilities available nearby our campus. The community of Oak Grove, the City of Milwaukie and Southeast Portland are home to a wide variety of medical professionals. If you choose not to drive yourself, transportation services are available for a fee.
